Form is a 52-year-old American film producer responsible for a number of recent big blockbuster movies and TV shows.

Originally from Long Island, NY, Form started his career in the industry as an assistant to legendary producer Jerry Bruckheimer on movies like Bad Boys and The Rock.

In the late '90s he worked his way up to being a full-time producer, which has been his role ever since. In November 2001, he co-founded the Platinum Dunes production company with filmmakers Michael Bay and Brad Fuller, and they've been responsible for a number of notable horror films and blockbusters since then.

Platinum Dune and Form's credits include: The Texas Chainsaw Massacre (2003), Friday the 13th (2009), The Purge (2013), Teenage Mutant Ninja Turtles (2014) and A Quiet Place (2018). He's also been behind successful TV shows like The Last Ship and Tom Clancy's Jack Ryan.

Daddario isn't the first Hollywood actress Form has been engaged to. In 2007, Form married Jordana Brewster, who he met on the set of The Texas Chainsaw Massacre: The Beginning. The pair were married for well over a decade, with Brewster filing for divorce in 2020, before finalizing it in June 2021.

In July, Daddario and Form made their red carpet debut, appearing together at the launch of her HBO show The White Lotus.

Alongside a picture of the happy couple smiling on a beach, she wrote about Form in the caption, calling him the "absolutely most wonderful man."

Daddario finished her sweet tribute by saying: "This is the love they talk about in poems and Ed Sheeran songs. Andrew- you are the greatest, most formidable love of my life. I couldn't be luckier."

Daddario's next project will be the movie Wildflower, which is a coming-of-age true story about a girl navigating life with an intellectually disabled parent.
